Endoscopic spine surgery on Kevin "Cub" Swanson, a UFC fighter
Kevin "Killer Cub" Swanson is a UFC fighter in the hall of fame. This video explains the work-up, diagnosis, and treatment of his low back pain and sciatic using an endoscopic technique by Dr. Sohrab Gollogly, MD. Cub had a combination of facet arthropathy causing low back pain and a facet cyst on the medial aspect of the facet joint causing inflammation and irritation of the S1 nerve, an important contributor to the sciatic nerve. The operation treated his sciatic pain by releasing the nerve from the adhesions and scar tissue that were tethering the nerve to the side of the spinal canal and preventing it from moving freely. A second procedure done at the same time, an endoscopic medial branch transection, interrupted the afferent nerve that carries pain signals from his facet joint to his brain. This procedure helped him avoid a more invasive surgery and return to combat sports without restrictions and after a relatively short recovery.

Did this develop after the fusion surgery or was it there at the time of it but the osteophyte did not get removed then to decompress the nerve root? It sounds like a wonderful relief for the person and sad if it was the case that they suffered because of the nerve compression not getting cleared at the time of earlier surgery if it was present then. Maybe you are just stating the order of surgical procedures rather than the initial causing the second which ultimately took the pain away because the pain was coming from the nerve root compression?
Did the person need the fusion or just the foraminotomy or both? If both are arising together is it possible to treat both in one surgical procedure? I am asking because I have stenosis at the nerve exit points and if injections do not work CDR or ACDF has been proposed and I would hope that the nerve roots could be freed from the bone spurs as well as the body of the disc being replaced. With thanks if you can clarify.

Can this be done on the cervical without the fusion? Or from the side? Dr. Gallogly, I was living in Monterey for years and got stuck in my homestate MN because of my neck. I want to get back to Monterey someday. But I don't want fusion. I have a disc/osteophyte complex, although have not had CT scan to confirm just how much osteophyte. Had been a fitness instructor on the peninsula for 6+ years. Help!

Thank you. To all annular tear sufferers - when you feel the time is right, start cycling. Exercise will free you from the pain. Work on your core and start cycling when you feel up to it. It works. Good luck. Don't give up.
what exercises for core you suggest for annular tear?
Planking! Start at one minute twice a day and increase slowly from there. Up to 3-4 mins. Do it before eating. Lose weight if you're overweight ... this takes pressure off your spine. Downward facing dog (gently) yoga exercise is good. Try to sit relaxed on a firm couch for ten mins a day. Move and turn slowly. The fissure takes up to two years to heal so try to be patient. Been on the fence of having this surgery for a few years now.. hurts like hell but its amazing what you can get use to. I see just as many people online and real life warning others NOT to do it because it makes things worse, as there are that say it works. If it made things worse for me I just dont know how I could even deal with that.. so im scared to pull the trigger. Not getting any younger though.. any advice out there? Im 42 with the l5-s1 annular tear. After 5 years from the injury the next mri showed that it had not healed at all. Could it still heal?

I call bullshit on much of this video. As someone that takes pain meds I have NEVER seen an advertisement by a pain med pharmaceutical company. Also, you make WILD generalizations that "many patients" are not educated about addictive properties of narcotics. It is an INSULT to the MILLIONS of patients that take pain meds responsibly AFTER trying other modalities of pain relief. This is a CLEAR CUT advertisement to advocate SURGERY over other choices. I have had major pain when I sit and bend forever since I was 18, I am now 33. I can palpate around my SI joint where the pain starts. I have had many cyst removed, all occuring on my left side of the body. MY major pain is on the left side. They did hernia surgery on me and found out I did NOT have a hernia. They assumed my pain was coming from my back. I have tried everything. How would I know I have some type of cyst like this there causing me this sort of pain? I need it to go away. Would an x-ray show? Or do I need an MRI

@@austinkunch710 I actually had an injection this past Monday where they did a diagnostic to see if L5/S1 is my issue and it confirmed it. So meeting with my surgeon next Wed to discuss fusing L5/S1. I hope I am pain free in 3 or 4 months. I’m not sure why simple pain medication found on the shelve is better than real severe pain medication? Is there an opiate crisis when approx 160 die per day (usually from going into heroin) compared to 9,850 die per day from alcohol? Yes the numbers are close to accurate. Alcohol is the 800 lbs gorilla in the room. The government must like to declare war on something easy to control?

yes they can. irritation of sciatic nerve root. numbness in foot toes, calf muscle spasm muscle twitch. Severe back and buttock pain.

i am fused from T1-C4. The back of my neck is so painful, It is always in a pain level of an 8-9! I have been going to physical therapy and when the therapist massages my neck it is so tender to the touch but feels amazing. After 2 minutes of her stopping all the pain comes right back to a 8-9 pain level! I cant lay in bed I have been sleeping in a recliner for about 5 years! I miss my bed so much! Also 4 months after surgery my left arm feels dead! And extremity painful! Doctor say to just keep going to physical therapy. I think the physical therapy is causing more damage. Before surgery it was my right arm that felt dead and extreme pain! now it switched, right arm is fine left arm is dead. But still have on and off every other hour both hands and arms BURN! Any advice would be so appreciated.
